Job Summary
To enhance patient care and safety, the full-time State Licensed Pharmacy Quality Specialist will collaborate with multidisciplinary healthcare teams, promote guidelines, provide education, and lead quality improvement initiatives in a remote setting.
Key responsibilities
- Ensures patient compliance with high-risk medications through various techniques and interacts with third-party payers to uphold quality measures
- Conducts medication reconciliation for patients transitioning to post-acute care and provides data on medication use to healthcare providers
- Participates in educational efforts and research projects related to quality and safety in pharmacy practice
Required qualifications
- Current licensure as a pharmacist in the relevant state(s) where services are provided
- Advanced degree (Master of Public Health or equivalent) preferred
- Completion of residency training or a minimum of three years of pharmacist experience
- Experience in quality improvement initiatives and medication management
- Ability to conduct and lead research projects in quality and safety arenas
