Key responsibilities
  • Support patient and provider engagement alongside the Clinical Care Team
  • Engage high-risk patients telephonically to schedule necessary appointments
  • Facilitate communication between the Care Team and provider offices regarding patient needs
Required qualifications
  • High School Diploma / GED
  • Must be 18 years of age or older
  • 2+ years of experience with medical terminology
  • 2+ years of medical or healthcare experience in a clinical or administrative role
  • 1+ years of experience as a State Licensed Pharmacy Technician
