Job Summary

A company is looking for a Program Manager - Chronic and Complex Care Management & Transitions of Care.

Key Responsibilities

• Oversee daily operations of the care management teams, ensuring efficient workflows and program goal achievement
• Ensure high-quality chronic care management and transitions of care, adhering to clinical pathways and documentation standards
• Develop strategies to enhance program performance and patient outcomes while supporting team development through training and coaching

Required Qualifications

• Bachelor's degree in healthcare or nursing (BSN required)
• Active state nursing license in good standing
• 3+ years of longitudinal clinical nursing care management experience
• 2+ years of experience in a clinical nursing managerial or leadership role
• Strong business acumen in value-based healthcare operations
