Job Summary
Providing telephonic support to nephrology practices, the full-time State Licensed Renal Care Coordinator will assess late-stage chronic kidney disease patients, educate them on treatment options, and coordinate care to ensure optimal transitions to dialysis.
Key responsibilities
- Assess patients' knowledge of late-stage CKD and provide education to facilitate informed decision-making regarding their health management
- Act as a liaison to ensure patients and families receive comprehensive information about treatment modalities and their benefits and risks
- Organize and manage the late-stage CKD patient population's education and transition to renal replacement therapy
Required qualifications
- Minimum of 6 months experience as a Renal Care Coordinator with a preference for 1-2 years of prior RCC experience
- Experience in renal transplant, dialysis, or chronic kidney disease patient care
- Strong organizational skills and the ability to work in a remote environment
- Availability to work during the hours of the time zone of supported practices
