Job Summary
Providing essential case management support, the full-time State Licensed Specialty Care Social Worker will engage in telephonic and video visits with patients, collaborate with interdisciplinary teams, and conduct psychosocial screenings to enhance patient care in a remote setting.
Key responsibilities
- Conduct telephonic and video visits with patients and caregivers to establish trust and provide support
- Collaborate with clinicians, nurses, and care coordinators to ensure cohesive, patient-centered care
- Perform psychosocial screenings and develop personalized care plans tailored to patients' needs
Required qualifications
- Master's degree in social work and active state license
- Minimum of 2 years of experience in the medical field
- Comfortable with technology and working in a fast-paced environment
- Bilingual in English and either Creole, Spanish, or Vietnamese is a plus
- Experience in geriatrics and health insurance (Medicare Advantage preferred)
