State Licensed Transplant Case Manager

Job Summary

Working remotely, the full-time State Licensed Transplant Case Manager will manage care for members with complex illnesses, ensuring all parties are informed of care plans while coordinating resources and support to reduce healthcare costs.

Key responsibilities

• Ensure all members and providers are fully aware of the comprehensive plan of care
• Engage with members and their care teams to facilitate care plan discussions
• Establish and hold accountable care management responsibilities across resources

Required qualifications

• Associate's or Bachelor's degree in Nursing
• 7+ years of clinical/acute care experience
• Advanced experience in targeted transplant programs
• Current, unrestricted RN license in the state of residence
• Experience managing multiple computer systems and tools
