State Licensed Transplant Nurse Advisor

Job Summary

Managing transplant patient needs remotely, the full-time State Licensed Transplant Nurse Advisor will perform intake and case setup for new referrals, facilitate communication among healthcare teams, and monitor patient health throughout the transplant benefit period.

Key Responsibilities

• Perform all intake and initial case set-up for new transplant patient referrals
• Communicate with referral sources, patients, and healthcare teams to gather necessary information for coverage determination
• Assess, plan, and coordinate the delivery of care for transplant patients while providing ongoing education about transplantation

Required Qualifications

• 4 Year / Bachelor's Degree in Nursing or a related field
• 5 years of relevant nursing experience, preferably in Transplant Case Management
• Active, current, and unrestricted state Registered Nurse (RN) License
• Broad knowledge base in disease states, particularly end-stage organ/tissue diseases
• Intermediate proficiency in Microsoft Office applications
