Job Summary
To support a growing virtual care team, the part-time State Licensed Virtual Care Nurse Practitioner will deliver holistic, evidence-based patient care services remotely, including assessment, diagnosis, treatment, and health counseling for patients aged 18 months and above, while ensuring a minimum of 9 hours of availability per week, including every third weekend.
Key responsibilities:
- Provide holistic, evidence-based care, including assessment, diagnosis, and management of health problems
- Evaluate and counsel patients on primary care, acute, and chronic conditions, along with health maintenance education
- Document all patient encounters in an EHR system and coordinate care with internal and external colleagues
Required qualifications:
- Current, unrestricted license in DE, FL, MA, OH, RI, or Washington D.C
- Minimum of two years of medically relevant experience or equivalent
- Completion of a Master's Degree level Family Nurse Practitioner program with current National Board Certification
- Ability to work independently and practice autonomously
- Proficiency in information management and technology
