State Licensed Virtual Care Nurse Practitioner

Job Summary

To support a growing telehealth initiative, the full-time State Licensed Virtual Care Nurse Practitioner will provide holistic, evidence-based patient care remotely, working weekends with a schedule from 2:30 p.m. to 11:00 p.m. while collaborating with a dedicated healthcare team.

Key responsibilities:

• Deliver holistic care through assessment, diagnosis, treatment, and management of health problems for patients aged 18 months and above
• Provide patient counseling on topics such as pregnancy prevention, STI prevention, and contraceptive care
• Document patient care accurately within an EHR and coordinate care with internal and external colleagues to ensure high-quality service

Required qualifications:

• Current licensure in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV
• Minimum of two years of medically-relevant experience or equivalent
• Completion of a Master's Degree level Family Nurse Practitioner program with National Board Certification
• Ability to work autonomously and effectively within a patient care team
• Proficiency in information management and technology
