State Licensed Virtual Care Nurse Practitioner

Job Summary

Providing holistic, evidenced-based care remotely, the full-time State Licensed Virtual Care Nurse Practitioner will assess, diagnose, and manage health problems for patients aged 18 months and above, while collaborating with a dedicated healthcare team during evening shifts that include every other weekend.

Key responsibilities:

• Deliver comprehensive patient care, including assessment, diagnosis, treatment, and health counseling
• Evaluate and provide care for primary, acute, and chronic conditions while educating patients on health maintenance
• Document patient interactions in an EHR system and coordinate care with internal and external colleagues

Required qualifications:

• Current unrestricted license in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV
• Minimum of two years of medically relevant experience or equivalent
• Master's Degree in Family Nurse Practitioner program with National Board Certification
• Proficiency in information management and technology
• Ability to work autonomously and collaborate effectively with healthcare professionals
