State Licensed Womens Primary Care Director

Job Summary

Providing medical leadership and collaborative clinical support, the full-time State Licensed Women's Primary Care Director will oversee patient care and population health metrics while leading a multi-disciplinary team to improve primary care for women across their lifespan, all in a fully remote setting.

Key responsibilities

• Clinically oversee an assigned patient population and drive population health programs for improved outcomes
• Collaborate with a team of APPs, RNs, and specialists to develop individualized care plans and lead care coordination meetings
• Facilitate cross-disciplinary reviews and support continuous improvement initiatives within the practice

Required qualifications

• Medical degree from an accredited US medical school
• Successful completion of an accredited US residency program in Family Medicine or Internal Medicine
• Active, unrestricted license to practice in the US with willingness to obtain licenses in all states
• 5+ years of experience practicing as an attending physician
• Certification with the American Academy of Family Physicians or American Board of Internal Medicine
