Job Summary

To promote patient skin health, the full-time State Licensed Wound Ostomy Nurse will collaborate with healthcare teams, conduct assessments, and implement individualized care plans for patients with wounds or ostomies while working remotely.

Key responsibilities

• Conduct nursing assessments of patients' skin health and identify risk factors for various age groups
• Collaborate with the healthcare team to formulate and document individualized nursing care plans
• Monitor treatment effectiveness and revise care plans based on evidence-based practices and patient needs

Required qualifications

• Current State Licensure as a Wound Ostomy Nurse
• Specialized knowledge in wound and ostomy care
• Experience in pediatric nursing or related healthcare field
• Ability to work collaboratively with healthcare teams and families
