Surgical Care Navigator
Job Summary
Supporting patient engagement for orthopedic patients, the full-time Surgical Care Navigator will conduct outreach, administer assessments, and coordinate care while working remotely.
Key responsibilities
- Conduct outreach to potential surgical patients and administer assessments to identify care goals and barriers
- Act as an advocate and educator for patients, assisting with appointment scheduling, transportation, and care coordination
- Serve as a liaison among patients, providers, and internal teams to promote evidence-based orthopedic care
Required qualifications
- Minimum 3 years of clinical experience in MSK, orthopedic, or value-based care preferred
- Experience with electronic health records and call center systems
- Ability to maintain HIPAA and security guidelines while handling protected health information
- Experience in fast-paced environments or startups
- Bilingual (Spanish/English) is a plus
