Job Summary
A healthcare company is in need of a Telecommute Assessment Intake Specialist in the Tuscarawas County Area.
Core Responsibilities of this position include:
- Working independently to manage a caseload of Ohio Home Care consumers
- Processing and providing recommendations for eligibility per program and company guidelines
- Conducting program assessments using program and company identified tools
Qualifications for this position include:
- This is a Field Based position
- Active license as RN
- 12 months experience in home and community based services within the past 5 years
- Knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs
