Job Summary
A healthcare company needs applicants for an opening for a Telecommute Bilingual English and Spanish Community Transitions Registered Nurse Case Manager in El Paso.
Individual must be able to fulfill the following responsibilities:
- Visiting members in the hospital to engage in post hospital program
- Making initial post hospital call within 3 days of discharge
- Identifying gaps or barriers in treatment plans
Skills and Requirements Include:
- Ability to travel several times per week to visit members within El Paso, TX
- Current, unrestricted RN license in El Paso, TX
- 5+ years’ experience as an RN in a clinical setting
- Bilingual in English and Spanish
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
