Job Summary
A healthcare company is filling a position for a Telecommute Bilingual English and Spanish LVN Case Coordinator in Laredo.
Individual must be able to fulfill the following responsibilities:
- Act as a liaison and facilitates the coordination ofbenefits, resources, healthcare providers, and institutions
- Work with themember to close clinical and pharmacy gaps and to mitigate non-clinicalbarriers to care
- Conduct in-person and/ortelephonic state mandated, health risk and other member assessments
Skills and Requirements Include:
- Travel: Yes, 50 % of the Time
- Current licensureas a Licensed Vocational Nurse (LVN)
- At least two years of related experience with Medicaid population
- Strong computer skills required
- Strong customer service skills required
