A healthcare company is filling a position for a Telecommute Bilingual Spanish Care Coach 1 in Fort Lauderdale.

Core Responsibilities Include:

• Identifying and resolving barriers that hinder effective care
• Ensuring patient is progressing by continuously monitoring patient care
• Assessing and evaluating member's needs and requirements

Qualifications Include:

• Must have a valid state driver's license and proof of personal vehicle liability insurance
• Bachelor's Degree with at least 2 years of relevant experience
• Registered Nurse (RN), licensed in the state of Florida
• Six years of professional human services experience
• Bilingual - English, Spanish - Must be able to speak, read and write in both languages
• Intermediate to advanced computer skills with Microsoft Word, Excel and Outlook
