Job Summary
A healthcare company is in need of a Telecommute Bilingual Spanish IRIS Consultant in Milwaukee County.
Candidates will be responsible for the following:
- Exploring a broad view of the participant’s life
- Assisting participants in identifying personal outcomes and ensuring those outcomes are being met
- Documenting all orientation and planning activities within the IRIS data system
Applicants must meet the following qualifications:
- Job related travel
- Bachelors degree in a social work or a closely related field (or four years of commensurate experience)
- One year of direct experience related to the delivery of social services to the target groups
- Knowledge of Long Term Care programs and familiarity with principles of self-determination
- Must possess a valid driver’s license and maintain adequate auto insurance
- Must meet all other listed company requirements
