Job Summary

A health insurance company that offers plans for individuals, families, and businesses has a current position open for a Telecommute Nurse Care Practitioner in Hauppauge.

Core Responsibilities of this position include:

  • Managing a panel of members in Long Island
  • Keeping a set patient schedule, treating patients, managing clinical escalations, and engaging in new products that are being rolled out for the program
  • Providing continuity of care to patients with a wide range of acuity from young/healthy to chronic conditions (DM, HTN, COPD, CAD, etc)

Applicants must meet the following qualifications:

  • Located in New York City and willingness to practice in-person
  • MSN or DNP from an accredited nursing school
  • APRN Board Certified as a Family Nurse Practitioner or Adult Nurse Practitioner
  • 5+ years experience delivering primary care
  • Licensed in New York
  • Professional Spanish-language fluency
