Job Summary
A healthcare company is in need of a Telecommute Care Coordinator Case Manager in Cabot.
Must be able to:
- Act as primary point of contact for members, guardians, and providers
- Engage members in care coordination activities through one-on-one
- Communicate with members about their medical needs
Skills and Requirements Include:
- Care coordination is provided primarily in person
- GED or High School Diploma required
- Minimum of one (1) year experience working with individuals with disabilities and/or mental health conditions
- Knowledge of Microsoft Word and Excel
- Valid driver’s license and reliable transportation
- Access to a reliable internet
