Job Summary
A healthcare company needs applicants for an opening for a Telecommute Care Coordinator in Santa Fe.
Individual must be able to fulfill the following responsibilities:
- Coordinates care of individual clients
- Provide face-to-face home visits
- Promote the appropriate use of clinical and financial resources
Must meet the following requirements for consideration:
- Able to perform face-to-face home visits
- 5 years post-licensure clinical experience
- Knowledge of various public and private services available for individuals with mental illness.
- Knowledge of hospital procedures governing the discharge of individuals with mental illness
- DL - Driver License, Valid In State - Other
- Associates: Nursing, Masters: Social WorkLicense and Certifications - Required
