Job Summary
An independent medical oncology and hematology practice is seeking a Telecommute Care Management and Patient Care Manager.
Core Responsibilities of this position include:
- Providing disease specific management
- Documenting assessment and interventions in real time in the electronic medical record
- Collaborating with the oncologist and clinic staff to assist in managing and coordinating patient care
Skills and Requirements Include:
- Must travel to Corporate Office for occasional meetings
- High School Diploma and a graduate of an accredited LPN program
- Minimum of 3 or more of years of experience in the healthcare field or clinic setting
- Knowledge of oncology required
