Job Summary

An independent medical oncology and hematology practice is seeking a Telecommute Care Management and Patient Care Manager.

Core Responsibilities of this position include:

  • Providing disease specific management
  • Documenting assessment and interventions in real time in the electronic medical record
  • Collaborating with the oncologist and clinic staff to assist in managing and coordinating patient care

Skills and Requirements Include:

  • Must travel to Corporate Office for occasional meetings
  • High School Diploma and a graduate of an accredited LPN program
  • Minimum of 3 or more of years of experience in the healthcare field or clinic setting
  • Knowledge of oncology required
