Job Summary

A health insurance company is filling a position for a Telecommute Care Manager.

Core Responsibilities of this position include:

  • Developing care plans
  • Reviewing insurance eligibility assessments
  • Providing chronically ill certifications and document claim records

Qualifications for this position include:

  • Ability to attend case conferences internally and with the client
  • Registered Nurse, or licensed Social Worker
  • Four years of experience in case management
  • Telephonic case management and/or home health care experience
  • Experience assessing and developing service plans
