Job Summary

A healthcare company is searching for a person to fill their position for a Telecommute Health Industry Care Manager.

Individual must be able to fulfill the following responsibilities:

  • Assessing the member’s current health status, resource utilization, etc
  • Coordinating services between Primary Care Physician (PCP), specialists, etc
  • Monitoring referrals made to community based organizations

Qualifications for this position include:

  • Current state’s RN license
  • Graduate from an Accredited School of Nursing
  • 2+ years of clinical nursing experience in an acute care setting
  • 1+ years of case management experience in a managed care setting
  • Experience with medical decision support tools, government-sponsored care programs
