Job Summary

A health insurance company is seeking a Telecommute Care Manager Telephonic Nurse 2.

Core Responsibilities Include:

  • Identifying and resolving barriers that hinder effective care
  • Ensuring patient is progressing towards desired outcomes
  • Creating member care plans

Qualifications Include:

  • Occasional travel (less than 20%)
