A healthcare company is filling a position for a Telecommute Care Manager Telephonic Nurse in the Central Region.
Core Responsibilities Include:
- Managing a member's physical, environmental and psycho-social health issues
- Identifying and resolving barriers that hinder effective care
- Ensuring patient is progressing towards desired outcomes
Must meet the following requirements for consideration:
- 3-5 years of clinical care experience
