Job Summary
A healthcare company is in need of a Telecommute Care Navigation Nurse Coordinator in Houston.
Candidates will be responsible for the following:
- Assessing patient condition, in conjunction with clinic staff
- Facilitating the coordination of care for patients
- Evaluating program processes; making recommendations to management
Position Requirements Include:
- Ability to perform site visits to clinics, medical offices, hospitals and/or patient homes
- Ability to maintain the integrity of Protected Health Information (PHI)
- Ability to comply with all departmental required data collection and auditing activities
