Job Summary

A non-profit organization has an open position for a Telecommute Care Navigator.

Individual must be able to fulfill the following responsibilities:

• Outreach via telephone to elderly and disabled patients
• Provide telephonic care coordination to patients with Medicare
• Work in close collaboration with physicians

Position Requirements Include:

• Knowledge of social services field
• Ability to write professional reports and correspondence
• Ability to work independently and as part of a team
• Ability to identify resources and utilize problem solving skills
• Excellent time management and organizational skills
