A healthcare company is searching for a person to fill their position for a Telecommute Care Transition Coach Registered Nurse in Cincinnati.

Individual must be able to fulfill the following responsibilities:

  • Follow member throughout a 30-day program
  • Ensure safe and appropriate transitions
  • Ensure member transitions to a setting with adequate caregiving

Qualifications for this position include:

  • 40-50% local travel
  • Graduate from an Accredited School of Nursing
  • 1-3 years hospital discharge planning or home health
  • Active, unrestricted State Registered Nursing license in good standing
  • Must have valid driver's license with good driving record
