Job Summary

A healthcare and retail pharmacy company is filling a position for a Telecommute Case Management Coordinator.

Individual must be able to fulfill the following responsibilities:

  • Utilize critical thinking and judgment to collaborate and inform the case management process
  • Provide care coordination, support and education for members through the use of care management tools
  • Conduct comprehensive evaluation of referred member’s needs/eligibility

Required Skills:

  • Bachelor's degree clinician required, with degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling)
  • Case management and discharge planning experience preferred
  • Managed Care experience preferred
  • Knowledge of chronic medical conditions/comorbidity, or behavioral health conditions
