Job Summary

A staffing agency is filling a position for a Telecommute Case Management Registered Nurse in Jackson County.

Candidates will be responsible for the following:

• Providing case management services to members with chronic or complex conditions
• Identifying, assessing and managing members per established criteria
• Performing ongoing monitoring of the plan of care to evaluate effectiveness

• Demonstrated ability to communicate, problem solve, and work effectively with people
• Excellent organizational skill with the ability to manage multiple priorities
• Work independently and handle multiple projects simultaneously
