Job Summary

A healthcare company is in need of a Telecommute Case Manager Register Nurse or Social Worker in Cook and Collar County.

Core Responsibilities of this position include:

• Completing clinical assessments of members per regulated timelines
• Conducting telephonic, face-to-face or home visits as required
• Facilitating interdisciplinary care team meetings and informal ICT collaboration

Applicants must meet the following qualifications:

• Local travel of up to 40% may be required
• Completion of an accredited LVN or Licensed Practical Nurse (LPN) Program OR Bachelor's or Master's Degree
• 1-3 years in case management, disease management, managed care or medical or behavioral health settings
• If licensed, license must be active, unrestricted and in good standing.
