Job Summary
A healthcare company is in need of a Telecommute Case Manager RN in Jacksonville.
Must be able to:
- Engage Members telephonically or face to face at Retail Centers, hospitals, etc.
- Plan, implement, coordinate and monitor Case Management activities
- Facilitate care for high risk members across the continuum of the health care experience
Skills and Requirements Include:
- Travel up to 30% for on-site member/provider/employer
- 2+ years related work experience or equivalent combination of transferable experience and education
- Registered Nurse - State Licensure And/Or Compact State Licensure
- Experience with influencing and motivating individuals to change health behaviors
- Knowledge and/or experience with population health
- Experience in home health care, utilization review, discharge planning, disease/ case management
