Job Summary

A professional employment agency and recruiting service has a current position open for a Telecommute Case Manager RN in Southern California.

Core Responsibilities of this position include:

• Working with members to create and implement an integrated collaborative plan of care
• Coordinating and monitoring Health Plan members progress and services to ensure consistent cost effective care that complies with Health Plan policy and all state and federal regulations and guidelines
• Providing case management services to members with chronic or complex conditions

Qualifications for this position include:

• 2 years of clinical experience with case management experience
• Active, unrestricted State Registered Nursing license
• Ability to multi-task with a desire to help others
• Ability to proactively identify members that may qualify for potential case management services
• Ability to conducts assessment of member needs by collecting in-depth information from Health Plans information system, the member, members’ family/caregiver, hospital staff, physicians and other providers
• Ability to develop and implement a case management plan in collaboration with the member, caregiver, physician and/or other appropriate healthcare professionals to address the member needs
