Job Summary
A healthcare company has a current position open for a Telecommute Clinical Pharmacist in Wauwatosa.
Individual must be able to fulfill the following responsibilities:
- Performing MTM telephonic pharmacotherapy consultations
- Completing a comprehensive medication review of prescription and non-prescription medications
- Completing pharmacotherapy assessment of patients' claims, past medical history and disease states
Must meet the following requirements for consideration:
- 50% of the time will be spent traveling to Providers in the greater Milwaukee area
- Pharmacy degree, and active, unrestricted license to practice as a Pharmacist in the state of Wisconsin
- 1+ years of experience as licensed Pharmacist
