Job Summary
A healthcare company is seeking a Telecommute Community Based Case Manager.
Core Responsibilities of this position include:
- Assessing, authorizing and providing ongoing case management
- Working with diverse population of persons with chronic illnesses or disabilities of all types
- Managing a caseload of Ohio Home Care individuals covering several counties
Applicants must meet the following qualifications:
- Field based
- Active license as RN, LSW, or LISW
- 12 months experience in home and community based services within the past 5 years
