Job Summary

A healthcare company has a current position open for a Telecommute Community Based Case Manager in the Franklin Area.

Must be able to:

  • Provide ongoing care planning and case management for individuals with serious mental illness and/or diagnosed chronic conditions
  • Maintain confidentially as related to patient information
  • Other duties as assigned

Skills and Requirements Include:

  • This is a home-based position that requires travel to individual's homes
  • Active license as RN, LSW, or LISW. OR Bachelor's degree in Social Work, Counseling, Psychology or related field
  • Minimum 3 years experience (post degree) working with persons with severe and persistent mental illness
  • Minimum of 1 year post degree experience working with individuals with diagnosed chronic conditions
