Job Summary

A healthcare company is filling a position for a Telecommute Community Based Case Manager in the Ross County Area.

Individual must be able to fulfill the following responsibilities:

• Assessing, authorizing and providing ongoing case management
• Managing a caseload of Ohio Home Care individuals covering several counties
• Helping a diverse population of persons with chronic illnesses or disabilities of all types

Required Skills:

• This is a field based position
• 12 months experience in home and community based services within the past 5 years
• Active license as RN, LSW, or LISW
• Knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs
