Job Summary
A health plans provider needs applicants for an opening for a Telecommute Community Care Coordinator in the McHenry Area.
Candidates will be responsible for the following:
- Facilitating ICT activities
- Conducting complete, timely and accurate program specific assessments
- Managing an assigned caseload
Position Requirements Include:
- This field based position
- Experience working with Inner City, Suburban and Rural neighborhood members
- Bachelor's degree with a concentration in healthcare or related field or AHM Designation is required
- Must also demonstrate the ability to work autonomously
