Job Summary

A company in the healthcare industry needs applicants for an opening for a Telecommute Community Case Manager in Darke County.

Candidates will be responsible for the following:

  • Working independently to manage a caseload of Ohio Home Care individuals
  • Assessing, authorizing and providing ongoing case management
  • Working with diverse population of persons with chronic illnesses or disabilities of all types

Skills and Requirements Include:

  • Field-based position
  • 12 months experience in home and community based services within the past 5 years
  • Active license as RN, LSW, or LISW
  • Knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs desirable
