Job Summary

A company that provides an innovative approach to supplementing traditional care coordination programs, we build relationships with your hardest to engage and highest utilizing members needs applicants for an opening for a Telecommute Community Coordinator in Phoenix.

Core Responsibilities Include:

  • Builds bridges between underserved individuals and the health and social services needed
  • Finds member populations that are difficult to locate (homeless, severe mental illness, substance use, medically underserved, in need of preventive services, etc)
  • Enrolls members into programs and services by effectively communicating their value

Required Skills:

  • Minimum of High School Diploma required
  • Minimum of 2 years of experience working in health, social or community services required
  • State-based certification may be required
