Job Summary

Core Responsibilities Include:
- Builds bridges between underserved individuals and the health and social services needed
- Finds member populations that are difficult to locate (homeless, severe mental illness, substance use, medically underserved, in need of preventive services, etc)
- Enrolls members into programs and services by effectively communicating their value
Required Skills:
- Minimum of High School Diploma required
- Minimum of 2 years of experience working in health, social or community services required
- State-based certification may be required
