Job Summary
A healthcare company is in need of a Telecommute Community Health Outreach Worker.
Individual must be able to fulfill the following responsibilities:
- Serve as a resource and consult to Care Coordination teams
- Serve as community liaison and maintain relationships with key individuals
- Serve as the direct personal contact in the community with members
Qualifications for this position include:
- Must possess a valid State driver’s license and travel is required
- Must reside in either St. Clair or Sanilac, Michigan
- Proven experience working with diverse populations, community or faith based organizations
- Knowledge of State and local social service agencies and community resources
- Knowledge of health education, motivational strategies, and empathetic working with the underserved
- Knowledge of assigned county and its residents
