A healthcare company needs applicants for an opening for a Telecommute Community Transition Coordinator.
Core Responsibilities Include:
- Guiding members in transitioning from higher levels of care
- Locating community services for members
- Arranging access to follow-up care
Must meet the following requirements for consideration:
- Ability to follow up with members in person or by phone after discharge
- Bachelors in a Healthcare related field
- Experience in a managed care environment, integrated medical and behavioral health
- Understanding of plan benefit structures and psychiatric/medical terminology
