Job Summary

A healthcare company needs applicants for an opening for a Telecommute Community Transition Coordinator.

Core Responsibilities Include:

  • Guiding members in transitioning from higher levels of care
  • Locating community services for members
  • Arranging access to follow-up care

Must meet the following requirements for consideration:

  • Ability to follow up with members in person or by phone after discharge
  • Bachelors in a Healthcare related field
  • Experience in a managed care environment, integrated medical and behavioral health
  • Understanding of plan benefit structures and psychiatric/medical terminology
