Job Summary

A major healthcare company is searching for a person to fill their position for a Telecommute Community Transitions Registered Nurse Case Manager in Boston.

Candidates will be responsible for the following:

  • Visiting members in the hospital to engage in post hospital program
  • Making a concerted effort to find members post hospital that are difficult to reach
  • Identifying gaps or barriers in treatment plans

Must meet the following requirements for consideration:

  • Access to reliable transportation and ability to travel to visit members within Boston, MA
  • Current, unrestricted RN license in state of residence
  • 5 years of experience as an RN in a clinical setting
  • Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
  • Must have a designated quiet workspace in your home
  • Must have access to install secure high speed internet
