Job Summary
An insurance company is in need of a Telecommute Community Transitions Registered Nurse Case Manager in Honolulu.
Must be able to:
- Identify gaps or barriers in treatment plans
- Visit members in the hospital to engage in post hospital program
- Educate members on disease processes
Skills and Requirements Include:
- This role will require visits to Queens Medical Center, Honolulu, HI multiple times per week
- Basic level of experience with Microsoft Word
- Current, unrestricted RN license in Hawaii
- 5 years' experience as an RN in a clinical setting
- Access to reliable transportation
