Job Summary
An insurance company is seeking a Telecommute Community Transitions Registered Nurse Case Manager in the Brooklyn Area.
Individual must be able to fulfill the following responsibilities:
- Providing patient education to assist with self-management
- Making concerted effort to find members post hospital that are difficult to reach
- Visiting members in the hospital and home to engage in post hospital program
Applicants must meet the following qualifications:
- Access to reliable transportation and ability to travel to visit members within Staten Island, NY
- Experience with Microsoft Word, with the ability to navigate a Windows environment
- 5 years of experience as an RN in a clinical setting
- Current, unrestricted RN license in New York
- Experience in geriatric chronic disease management
