A major healthcare company is searching for a person to fill their position for a Telecommute Community Transitions RN Case Manager in Brooklyn.
Core Responsibilities of this position include:
- Providing patient education to assist with self-management
- Educating members on disease processes
- Encouraging members to make healthy lifestyle changes
Applicants must meet the following qualifications:
- Access to reliable transportation and ability to travel to visit members within 30 miles
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
- Must have a designated work space and access to install secure high speed Internet via cable in home
- 5 years experience as an RN in a clinical setting
- Current, unrestricted RN license in New York
- A background in case management, community care or home care
