Job Summary
A healthcare provider is in need of a Telecommute Community Transitions RN Case Manager in Brooklyn.
Must be able to:
- Visit members in the hospital in order to engage in a telephonic post discharge program
- Find members post-discharge that are difficult to reach
- Identify gaps or barriers in treatment plans
Qualifications for this position include:
- Access to reliable transportation and ability to travel to visit members within 30 miles of (NY Presbyterian Hospital)
- Current, unrestricted RN license in New York
- 5+ years’ experience as an RN in a clinical setting
- A background in case management, community care or home care
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
- Must have a designated work space and access to install secure high speed Internet via cable in home
