Job Summary
A healthcare company is searching for a person to fill their position for a Telecommute Community Transitions RN Case Manager in Puyallup.
Must be able to:
- Educate members on disease processes
- Provide patient education to assist with self-management
- Make initial post-discharge call within 1- 3 days of discharge
Applicants must meet the following qualifications:
- Travel to visit members within a hospital setting in Puyallup, WA several times a week
- 5 years' experience as an RN in a clinical setting
- Current, unrestricted RN license in Washington
- A background in case management, community care or home care
- Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
- Must have a designated work space and access to install secure high speed Internet via cable in home
