Job Summary

A staffing and training provider is filling a position for a Telecommute Complex Case Manager Registered Nurse in San Francisco.

Core Responsibilities Include:

• Performing assessment, planning, facilitation and advocacy through collaboration with health care resources
• Developing multi-disciplinary care plans to address identified member problems
• Participating in weekly Multidisciplinary Care Team Meetings and be able to present complete medical cases

Required Skills:

• BSN or MSN required
• Registered Nurse license, active and unrestricted licensed in the State of California before start of work
• Certification in case management or willing to become certified within 12 months of hire or a date agreed to by supervisor
• Medi-Cal and Medicare regulations needed
• Three years experience in clinical case management required
• All other requirements necessary for this position
